         The defendant pleaded guilty on Count One of the Indictment. Accordingly, the court has adjudicated that the defendant is guilty of the following offense:

         The defendant is sentenced as provided in pages 2 through 6 of this judgment. The sentence is imposed pursuant to the Sentencing Reform Act of 1984.

         Counts Two, Three and Four of the Indictment are dismissed on government's motion.

         IT IS ORDERED that the defendant must notify the United States Attorney for this district within 30 days of any change of name, residence, or mailing address until all fines, restitution, costs and special assessments imposed by this judgment are fully paid. If ordered to pay restitution, the defendant shall notify the court and United States Attorney of any material change in the defendant's economic circumstances.

         IMPRISONMENT

         The defendant is hereby committed to the custody of the United States Bureau of Prisons to be imprisoned for a total term of SEVENTY-SEVEN (77) MONTHS as to Count One of the Indictment with credit for time served as calculated by the Bureau of Prisons.

         The Court recommends to the Bureau of Prisons:

         1. 1st choice of incarceration - FCI Coleman, FL

         2. 2nd choice of incarceration - closest to Tampa Bay as possible

         3. Medical examination for variety of health problems and need for appropriate medication.

         4. 500 hour substance abuse counseling. Appropriate mental health counseling for bi-polar condition. See medical reports.

         5. Both academic and vocation evaluations. Defendant has vocational interest in jewelry design courses and business administration related to that vocation. Anger management counseling regarding discipline
