         The defendant pleaded guilty to Count One of the Indictment. Accordingly, the court has adjudicated that the defendant is guilty of the following offense:

         The defendant is sentenced as provided in the following pages of this judgment. The sentence is imposed pursuant to the Sentencing Reform Act of 1984.

         Count Two of the Indictment is dismissed on the motion of the United States.

         IT IS ORDERED that the defendant shall notify the United States Attorney for this district within 30 days of any change of name, residence, or mailing address until all fines, restitution, costs and special assessments imposed by this judgment are fully paid. If ordered to pay restitution, the defendant shall notify the court and United States Attorney of any material change in the defendant's economic circumstances.

         Original Date of Imposition of Sentence July 13, 2012.

         IMPRISONMENT

         The defendant is hereby committed to the custody of the United States Bureau of Prisons to be imprisoned for a total term of EIGHTY-SEVEN (87) MONTHS as to Count One of the Indictment.

         The Court makes the following recommendations to the United States Bureau of Prisons:

         1. Incarceration at Coleman, FL or somewhere in the state of Florida

         2. UNICOR to send support for four minor children

         3. Immediate medical exam for cardiac arrhythmia; defendant has pain and other issues in extremities. Provide treatment and medication.

         4. Substance abuse counseling primarily for alcohol but also drugs.

         5. Provide education to defendant speaking English language. Provide all elementary and secondary education. Procure GED.

         6. Vocational training in electrical trade, welding, auto mechanics, computers ...
