         Defendant is sentenced as provided in pages 2 through 6 of this judgment. The sentence is imposed pursuant to the Sentencing Reform Act of 1984.

         Counts 2, 3, 4, 6, 7, 9, 10, 11, 12, 13, 15 and 16 are dismissed on the motion of the United States.

         IT IS ORDERED that Defendant must notify the United States Attorney for this district within 30 days of any change of name, residence, or mailing address until all fines, restitution, costs and special assessments imposed by this judgment are fully paid. If ordered to pay restitution, the defendant shall notify the court and United States Attorney of any material change in the defendant's economic circumstances.

         IMPRISONMENT

         Defendant is hereby committed to the custody of the United States Bureau of Prisons to be imprisoned for a total term of 24-MONTHS, term consists of 24 months as to Counts 1, 5, 8 and 14, all such terms to run concurrently.

         The Court makes the following recommendations to the Bureau of Prisons:

• The Court recommends to the Bureau of Prisons that the defendant participate in the Institution Hearing Program of the Federal Bureau of Prisons to determine his removal status while incarcerated.
• Incarceration in a facility close to home (Naples, Florida).

         Defendant is remanded to the custody of the United States Marshal.

         SUPERVISED RELEASE

         Upon release from imprisonment, you will be on supervised release for a term of 4-YEARS, such term consists of 1 year Supervised Release as to Counts 1, 5, 8 and 14, all such terms to run consecutively.
